Q&A hcc coding

I am new to coding as I am a hcc coder. I am struggling with trying to get a firm grasp of problem lists vs coding from the assesment because I know they 2 diff guidelines to go by. Like what is needed for the problem lists vs the assesment. I try my best not to overcode, but I centainly dont want to undercode! I know everyone codes diff but Im tired of second guessing myself.

Another is specficity.....major depression vs depression if both are on problem list, they are onalot of meds? i know the ratio is 1:1 but do you still code both if both are mention in say 2 other areas of the chart?
I swear Im over thinking things?!?!
